Department: Child Development Center (Hulhumale’ Hospital)

MINIMUM QUALIFICATION & REQUIREMENT

Bachelors Degree in relevant field with minimum 2 years’ experience in relevant field.

SCOPE OF WORK

• Complete psycho social assessment to determine a picture and understanding of the child/family as unique individuals who will respond in their own way to the diagnosis
• Responsible for promoting patient-centered care by evaluating psychosocial needs, providing community service coordination for identified patient populations, and providing support and crisis intervention where indicated. The social worker acts as a liaison between the patient and community resources/post-acute care providers. The social worker creates and facilitates timely implementation of discharge plans and arranges post-acute interventions identified in the plan of care. The social worker facilitates patient movement across the continuum of care and identifies, addresses, and tracks barriers to patient throughput.

SALARY PACKAGE:

Basic Salary: MVR 7035.00

Service Allowance: MVR 2500.00

Risk Allowance: MVR 100.00 per day attended

Technical Core Allowance: 28% of basic salary
